Lodge online
- You can save the form at any time, then complete and submit it later
- Try to fill out all sections of the form. Please answer all questions and tick the relevant boxes. Questions marked with this symbol * are mandatory and must be completed, unless you see an instruction to go to another question. Leaving a question blank may delay the processing of your claim
- Give as much detail as you can in the form to help us assess your claim for GEERS assistance
- You can attach up to 25MB electronic copies of supporting documents (such as payslips, a signed contract of employment, letter of termination, timesheets, payment summaries and/or separation certificate):
- Or, you can send the extra supporting documents by post. Keep a copy of the supporting documents for your own records. Do not send original documents as they will not be returned. Please write your name and date of birth on all documents so we can match them to your GEERS claim and mail to:
